Valley Health Partners (VHP) primary and family care providers deliver caring, compassionate and comprehensive health care for people of all ages. Their education prepared them to manage the health concerns of infants, children, adults and older adults.

Regular visits to your primary or family care provider helps to keep you healthy. During these visits, which usually occur annually for older children and adults and more frequently for young children, the health care provider identifies your risk factors for disease, then helps you take steps to avoid developing a health problem. Any health issues have a better chance of being caught early when treatment is most effective.

A full range of services is available to people who become patients at VHP clinics. 

Primary and family care providers may refer people to other services available at VHP that are designed to help them get and stay healthy.

When you’re sick, your primary or family care provider will manage your health care. He or she may help you see a specialist, receive tests or treatments.

Seeing your health care provider for a wellness checkup or physical exam helps you to stay healthy and prevent disease. It also helps your health care provider to get to know you and assess your health care needs.

Your VHP primary or family care provider wants to care for you when you’re sick. Call the clinic or schedule a virtual visit.

How else can my primary or family care doctor help me?

A primary or family care provider can refer you to a specialist when you’re sick. He or she can also refer you to the education classes or other services at VHP.
