What are the special services offered?
- Our social services team helps break down barriers by connecting patients to needed resources in the community, including transportation, food, housing, mental health services, financial resources, education, child care services and more.
- Behavioral health specialists assess and assist with behavioral health needs.
- Our team of nurse care managers provides ongoing care coordination, care planning and support to those facing complex medical needs, working in partnership with your primary care physician and/or specialists to achieve optimal health.
- Our transition of care team connects with patients after their hospital or emergency room visit to ensure a smooth transition back to normal life.
- Our clinical pharmacists work closely with primary care physicians, performing medication reviews to help you reach your goals of therapy, help with obtaining your medications and more.
- Our team of financial counselors evaluates our patients’ financial needs and connects them to programs to assist with medical bills payment and beyond.
- We are also proud to offer Spanish-speaking medical interpreters within our practices and virtual interpretation in more than 40 languages.
